REGISTRATION FEES
Total Team fee: $1430 + GST ($800 deposit due with application - $701.50 due Nov. 18, 2008)
$50 NSF surcharge (if applicable)
MONDAYS ONLY Total team fee: $1399 + GST

Balance of payment must follow schedule noted above. Registration will only be confirmed upon complete application package and full deposit payment.
ADDITIONAL FEES
Referee fee: $15/team/game
OSA Player Insurance fee: $15/player per player on team roster.
** All participants MUST be registered.
Teams may play double-headers
Changes to proposed schedule may be affected by number of teams entered in a Division
All games kickoff from 6pm (5pm Sundays only ) to 11pm
